We welcome adults with diabetes who are motivated to get their diabetes under control, especially those individuals who are looking for a calm, supportive environment where voices are heard and health related questions are answered.
Visit includes a thorough Diabetes Wellness assessment and evaluation including evaluation of diabetes and blood pressure status. Intention of visit is to develop a goal driven client-focused Diabetes Wellness plan. Ongoing obstacles to goal attainment will be addressed. Diabetes focused education and support will be provided.
It depends. We do accept some medical insurance and are adding insurances that we accept all the time. We will check your insurance and let you know if we can accept your insurance for payment. Check your medical coverage.
