Through the Diabetes Wellness Practice, Dr. Zaldivar will work with you to reach your diabetes wellness goals by developing a medical treatment plan that makes sense to you in your everyday life and by answering any diabetes-related questions that you and those who assist you with your diabetes may have about managing your diabetes.
Caregivers are encouraged to join you during your visit or to call in with their questions during your visit. You will work towards reaching your diabetes wellness goals in a peaceful, nonjudgmental and supportive environment.
Initial Visit
We are located in Wellness Space US, 8804 Third Avenue, Brooklyn NY 11209
All new clients to the Diabetes Wellness Practice must be scheduled for Diabetes Wellness Initial Visit first.
Visit includes a thorough Diabetes Wellness assessment and evaluation including evaluation of diabetes and blood pressure status. Intention of visit is to develop a goal driven client-focused Diabetes Wellness plan. Obstacles to achieving optimal diabetes goals will be explored.
An individualized diabetes education plan will be developed with an emphasis on answering client questions regarding diabetes care.
Prompt Payment Option
Clients wanting to take advantage of Prompt Payment option will be charged:
$175 for Diabetes Wellness Initial Visit
$50 for Diabetes Wellness Follow- Up Visit
Due at the time of visit. Credit Card or Venmo accepted.*
Clients choosing this option should complete intake form .
*Visits may be reimbursable through Flexible Spending Accounts (FSA). Will provide receipt of visit.
